Program Forms

Applicants for the diagnostic medical songraphy program must complete the appropriate forms to be considered for acceptance.

• Associate Degree Application Package
  

  This application package is for applicants who meet the pre-requisite criteria for the Associate Degree Track.

  ALL forms must be filled out completely and turned in with your application. Incomplete forms will not be accepted.

• Certificate Track Application Package
  

  This application package is for those who have already completed:

  • An associate degree in an allied health program which is medically related and involved human patient care with a 3.0 GPA or better; curent/actuve registration/license in that particular allied health specialty. Note: recent graduates may apply before they take national registry examinatins provided that they schedule and pass their national examination before start date of the program.
  • College Algebra (MATH 121) or equivalent

  OR

  • A Bachelor's degree in a biological or health-care related science with a cummulative GPA of at last 3.0.
  • Must include College Algebra, Medical Terminology, two semesters of Anatomy & Physiology and General Physics.
    

   

  All forms must be filled out completely and turned in with your application. Incomplete forms will nto be accepted.

• Re-Application Form
  

  Applications will be kept by the program for no longer than one year. Students who have previously applied for the program within one year may use this form. Additional forms are not required unless there has been a change in status or new pre-requisite criteria.

  Students who have previously applied and chose to pick up their application documents must submit new forms to the program to be considered.
